Form I-9 – Employment Eligibility VerificationPlease complete Form I-9 by following the steps below: Step 1: Download a copy of Form I-9. Step 2: Complete Section 1, sign and date the form (this is a fillable form). Step 3: For Section 2, you are required to present original document(s) to show your identity and employment authorization. You will need to present an original document from List A -OR- two separate documents – one from List B and one from List C. See Page 3 of Form I-9 which provides a full list of acceptable documents. Step 4: Present your original document(s) to an authorized WELS representative where you are located so they can physically examine each document verifying it appears reasonably genuine and relates to the employee presenting it. A WELS representative can be another WELS/ELS Pastor, a WELS called workers, a church secretary, etc. IMPORTANT: Reviewing or examining photocopied documents or viewing documents via webcam is not permissible. Step 5: Have the WELS representative complete Section 2 by recording the specific document(s) information on the form either under List A -OR- List B and List C. Step 6: Have the WELS representative sign and date the form certifying that they have examined your document(s) and they appear to be genuine. Step 7: Upload a copy of the completed Form I-9 using the secure upload found on the Financial tab under Upload Documents.
